Workers’ Compensation

The District provides Workers’ Compensation benefits to employees who experience an injury/illness arising out of employment.

To report a workplace injury/illness:

Report the injury to your supervisor

For life-threatening injuries: 911 should be called immediately, and then another employee should notify the supervisor, Company Nurse, and Jennifer Taylor, Risk Manager, at 707/399-5083 as soon as possible.

For all other injuries: FSUSD participates in a Medical Provider Network for the treatment of workplace injuries/illnesses. Employees in need of medical treatment may be seen at the following locations*:

*Failure to report an injury to Company Nurse prior to arriving at the clinic may result in delays in receiving medical treatment.

Employees have the option of pre-designating their personal physician to treat work-related injuries & illnesses.

Americans with Disabilities Act

The District endeavors to provide equal access to all of its programs, services, and activities. If you need special assistance, accommodations, or other modifications to participate in any of the District’s programs, services, or activities, including the pre-employment process or public meetings, complete the Reasonable Accommodation Request Form and fax it to Jennifer Taylor, Risk Manager (ADA Coordinator), at 707/399-5139.

Ergonomic Evaluations

Ergonomic evaluations are available to District staff upon request. The evaluation process is a combination of workstation and/or task assessment, equipment adjustment, habit modification, and education. The desired outcome of the ergonomic evaluation is to better fit the work environment to the worker to reduce risk factors associated with repetitive motion injuries, as well as educate employees on proper posture and body mechanics to perform work tasks in a safer manner. To request an ergonomic evaluation, complete the Ergonomic Evaluation Request Form and fax it to Jennifer Taylor, Risk Manager, at 707/399-5139.

Employee Assistance Program

The District has an Employee Assistance Program that offers clinical counseling, work and life services, online member services, and other members-only benefits. For more information on our Employee Assistance Program, including the District’s online access code, look for the EAP brochure in your site’s break room. Or you may contact Jennifer Taylor, Risk Manager, at 707/399-5083. Your request for access and benefit information will remain confidential.

Safety Program

The District has an extensive Safety Program in compliance with State and Federal Occupational Safety & Health Regulations. The goal of the Safety Program is to identify and minimize safety hazards that may exist around District facilities, as well as educate staff and promote safe work practices. A Safety Committee comprised of Classified and Certificated staff meet monthly during the traditional school year to discuss specific accidents and potential safety hazards. The Committee also helps coordinate the annual Safety Grant Program.

To report a safety hazard, complete the Safety Hazard Reporting Form and fax it to Jim Walker, Safety Technician, at 707/425-8641.
